NHS Health Scotland Surveys

Community Planning Partnership external web user survey

The purpose of this survey is to invite CPP views on the developing Health Scotland CPP website. NHS Health Scotland is a national Health Board, working with and through public, private and third sector organisations to reduce health inequalities and improve health. Our primary role is to lead the co-production and improvement needed to translate knowledge of what works to reduce health inequalities and improve how knowledge is turned into action.

The website is our attempt to gather all Health Scotland existing, standalone resources, tools and information into one place. We believe these products will be useful to different members of the partnerships in your role in reducing health inequalities. We recognise that there will be other things we need to develop and add, however this website is a starting point.

This survey will take you approximately 10-15 minutes to complete. We will use your feedback to shape the website and develop new aspects of our offer going forward.
There are 16 questions in this survey.
  A note on privacy
This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven't) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
